Application for rate reduction for holders of Low Income Health Care Card form

The rate reduction is only available to persons who hold a Low Income Health Care Card with the identifier ‘LI’ or ‘LIC’ shown on the card.

You will need to upload a copy of your Low Income Health Care Card with this application.
 
To be eligible for a rate reduction for the current financial year rates, applicants must: 

• Have a current Health Care Card granted due to low income
• Be responsible for the payment of rates for their principal place of residence at the time of application. 

Please note: you are required to apply for this rebate each rating year. Rebates will only be applied for this rating year, not previous years.
